As a Clinical Case Manager, you will work in-home and community based setting in Moorestown, Burlington County, NJ. 

Schedule: Monday-Friday: varied shifts according to program needs with a 30 minute unpaid lunch break; some on call responsibilities and weekends as needed

Competencies:

The incumbent must be able to demonstrate: Respect for children and families; An understanding of emotional disturbance; An understanding of the principles of collaborative community-based care; Have knowledge about the differential characteristics of serious emotional disturbance; Have knowledge about the role of psychiatric medication in treating disorder; Have knowledge of developmental needs of children; Have an understanding of the effects of stressful life events including abuse and/or trauma; Involves youth and family in all aspects of treatment planning; Has the ability to teach both simple and complex skills; Have knowledge of a range of strategies applicable to population; Ability to identify family support resources and provide advocacy for their use; Communicates understanding of unique issues families face; and have the ability to provide support while setting clear limits.

Responsibilities:

  • Assess the family’s strengths and needs and provide therapeutic services
  • Provide individual, group and family therapy
  • Develop Individual Service Plans in collaboration with families and other staff members
  • Provide psychoeducation and clinical case management
  • Conduct assessments as needed
  • Provide crisis intervention as needed
  • System/service provider collaboration
  • Promote service provision and environment that is trauma informed, is recovery-based and culturally responsive to families

Qualifications:

  • Masters Degree in social work, human services, counseling or a related field required; MSWs must have LSW

  • Valid NJ clinical license (LAC, LCSW or LPC) preferred;

  • Bilingual preferred.

  • Valid Driver's License required.
